Seniors Community Connector
Offered in partnership with United Way BC, the Social Prescribing program aims to help seniors improve their health and well-being by connecting individuals to meaningful activities and resources. Through the Community Connector, participants can receive help in accessing information and community services, or participants can choose to co-produce a comprehensive plan involving multiple areas of support with follow-up from the Community Connector. Some areas of service can include, but are not limited to; nutritional supports, physical activities, social, volunteer or creative opportunities and caregiver supports.
This program is for older adults (65+) living in Kamloops who are experiencing:
- social isolation or loneliness
- major life events such as loss of a spouse
- physical inactivity
- chronic health ailments
- caregiver fatigue
- poor nutrition and/or food security concerns
- the desire or need to connect with community resources or services
- wanting to make some changes with current life situation but just don’t know where to start
Who Is Eligible?
Seniors 65 and over living in Kamloops.
How to Refer
Referrals can come from health care providers, community service agencies, family or individuals can self-refer.
